Issues with the submitting ticket via cc

I am having the following issues while submitting the ticket through cc:

  1. It doesnt get assigned to the right customer. currently, it doesnt assign the ticket to any customer.
  2. The portal doesnt pick up the subject line and the priority from the ticket form. all the information is lost and it changes the subject line to recommended performance monitoring triggered on.
